An employee was investigating a possible malfunction in a vacuum cleaner system for removing powder paint from paint booths. While his right hand was in the discharge outlet of a rotary airlock valve on a dust collector, the blade inside the valve caught his glove and pulled his hand further in. He suffered partial amputations to two fingers, losing half of each one.

An employee was preparing to repaint the indoor pool. The pool had been drained and a scaffold was erected inside of the pool. The employee leaned over the railing to drop a broom down to another employee working below when the midrail on the scaffold failed causing the employee to fall 12 feet before striking cross bracing and then fall another three feet to the bottom of the pool. The employee was hospitalized for a broken wrist and internal abdominal injuries.

As he was descending from the platform, he swung under the top rail to the floor and momentarily lost his balance. His right hand then entered the maintenance access opening on the backside of the box former, resulting in a partial digit amputation at the middle phalanx of the right index finger.
